Homeschool Membership Form

1.Proof of homeschooling is required at the time of purchase. Acceptable documentation may include a state-issued acknowledgment letter or other official verification of homeschool status. Membership cannot be issued without verification. 2.Membership good for one year from date of of purchase. 3.Each time you come to The Archway for Homeschool purposes, notify the cashier how many have returned and complementary tickets will be given each time. 4.Only members of your Homeschool are allowed to use the Homeschool membership. Additional family members or friends not in your homeschool must purchase tickets at the standard ticket price.Non-homeschool individuals are not allowed to use this membership.

Acceptable documentation may include: 1. A state-issued acknowledgment letterfrom the Nebraska Department of Education (or your homeschool state) 2. A screen capture or printout showing your registration with the State of Nebraska (or your homeschool state) 3. A copy of your letter of intent to homeschool and/or a confirmation email/letter from your school district 4. Membership card or documentation from a recognized homeschool association/coop

I hereby affirm that all homeschooling documentation submitted to the Archway is true, correct, and current as of the date of submission. I certify that this information has been provided in good faith and understand that I am responsible for promptly notifying the Archway of any changes and for submitting updated documentation as required.
